National medical group seeking Critical Care Physicians to do in home and virtual visits with patients. This medical group has a unique care delivery model to deliver better patient care for those patients most in need living in ethically and geographically diverse communities across the US.

Our clients core care teams are multi-disciplinary and include Community Health Workers (CHW), Registered Nurses (RN), Advanced Practice Providers (APP- NP/PA), and Advanced Practice Providers (CCP). They are supported by outreach, care coordination, and specialized resources such as palliative care and psychiatric clinicians, as well as Social Workers (SW), Clinical Pharmacists, and Registered Dieticians (RD).

Their care teams provide comprehensive medical, behavioral, and social care to vulnerable senior and adult patients through virtual and direct in-home care. They deliver this care both as the assigned primary care specialist and/or the attributed home-visiting specialist.

This full-Time Critical Care Physician will spend at least 80% of their time in direct patient care in-person and virtually (from a home office). A typical day could include up to 4 in person home visits (50-mile radius), including unplanned, urgent, and/or emergent in-home visits as needed to meet patient needs. The rest of the time will be spent coordinating care with the rest of the team (nurses, social workers, dieticians, community health workers, NP’s) from a home office.

  • Partner with our Talent Acquisition Team to recruit and hire the best care team members, orienting them toward a value-based model of care that includes a strong focus on home- based care solutions that take into account behavioral health conditions and social determinants of health
  • Build a successful market medical practice that carries an excellent reputation among communities of patients and the health care delivery ecosystem
  • Meet or exceed market-level quadruple aim targets, including Team Experience/Joy In Work; Patient Experience- Net Promotor Score; Better Care-admission, readmission, quality metrics; Lower Total Cost of Care- MLR, total cost of care reduction
  • Provide coaching and mentorship to a multi-disciplinary care team
  • Data-driven clinical oversight of multidisciplinary teams in partnership with Clinical Service Managers and Complex Care Physicians
  • Deliver timely healthcare services tailored to the needs of individual patients, and in compliance with our high-touch care delivery model
    Direct delivery of home- and community-based care (preventive, acute, chronic care management)
  • Communicate closely with other healthcare providers so that primary and specialty care is well coordinated, including the use of virtual consult platforms
  • Ensure accurate and thorough clinical documentation for all patients
  • Lead clinical education and quality improvement activities and meetings at the market level

Requirements for Critical Care Physician

We are searching for a special breed of health care professional who embodies the following qualities and characteristics: heart and commitment to serve vulnerable populations, passion and perseverance to achieve long-term goals (a.k.a. grit), team-based and social determinants of health orientation, and embrace change in a rapidly evolving health care delivery system. Flexible and dynamic, this self-starting individual will be a creative problem solver with a proven track record of successful implementation of innovate health care delivery solutions. They must possess excellent time management and organizational skills, with the ability to prioritize and multi-task. Additional qualifications include-

  • Graduate of an accredited health professional school
  • Board Certified American Association of Nurse Practitioners (AANP), American Nurses
  • Credentialing Center (ANCC), Physician Assistant (PA)
  • Experience and/or training in Emergency Medicine, Gerontology, or Palliative Care is
    preferred
  • At least two years of clinical experience delivering care to complex patients preferred,
    including in the home and skilled nursing facility settings
  • Strong knowledge of best clinical practices as they relate to population health
  • Strong knowledge of best clinical practices, particularly in the urgent and post-acute care settings
  • Must be skilled in delivering care across medical, behavioral, and social needs, particularly for patients living with mental illness and substance use disorders
  • Demonstrated ability to collaborate both internally and externally with multi-disciplinary care team members
  • Active license in the indicated State, or ability to obtain one within 90 days
  • Medication-assisted treatment certification preferred if State permits (if not, expectation will be to become certified within first 120 days)
  • Where indicated by State law/regulations, must have current state and federal DEA certification
  • Current Certification in AHA or ARC Basic Life Support for health care providers required for employment
  • Working knowledge of computers and ability to document effectively and efficiently in an electronic medical record system
  • Driver’s license requirements: licensed for a minimum of 3 years without restrictions; no DUI or other felony driving conviction in the past 7 years
