typical roles and responsibilities for RN Case Manager

  • Access patient needs and determine priorities for disease management programs and services.
  • Facilitate the review and selection of appropriate patient education resources and tools and strategies for patients.
  • Conduct disease management assessments using standardized instruments and documents in EMR and/or care management platform as appropriate.
  • Act as a resource person for patient, provider, care center staff and the community.
  • Develop repository of best disease management practices for the disease management team and facilitate educational opportunities for patients.

Requirements for RN Case Managers

  • Licensed RN; MS degree preferred in state of practice
  • 3+ experience as a case manager for patients with chronic medical conditions, these could include congestive heart failure, coronary artery disease, hypertension, diabetes, COPD or asthma.
  • Demonstrates passion for extraordinary patient service and personality for developing close relationships with patients.
  • A track record of accomplishment in managing a case-load of medically complex patients
