Developing referrals for private pay nursing services with key referral sources including: hospital discharge planners, social workers, case managers, and physicians, as well as community Assisted Living, and Rehab facilities.

Requirements

• Established relationships with referral sources 
• Experience in home care and related services
• Polished presentation skills 
• Strong computer-based skills 
• CRM software experience
• Clinical background a plus, not required
• Bachelors Degree
