Responsibilities:
· Supervise day-to-day clinical and office operations; remain available to staff and patients during business hours.
· Ensure all services meet professional standards, agency policies/procedures, and state/federal regulatory requirements.
· Coordinate patient referrals and care with the interdisciplinary team.
· Oversee the development, implementation, and updating of individualized plans of care.
· Evaluate the quality of clinical care and continually assess patient needs.
· Collaborate with clinicians and physicians on patient transfers, discharges, or changes in condition.
· Manage OASIS documentation, edits, discharge processes, and related data.
· Monitor key performance indicators (KPIs), workflows, and escalations.
· Guide clinicians on emergency care protocols.
· Provide training, supervision, coaching, and mentoring to clinicians (including contract staff); assign patients/staff based on competence.
· Act as QAPI (Quality Assessment and Performance Improvement) owner; participate in QA activities, investigate incidents, analyze trends, and implement corrective actions.
· Support accreditation readiness, QA audits, HHCAHPS surveys, and emergency preparedness planning.
· Serve as backup for other roles (e.g., Patient Care Manager) and act as leader in the absence of the Territory Administrator.
· Manage operational aspects in collaboration with the team to support exceptional home health care.
