We have an extraordinary opportunity for a compassionate and skilled Registered Nurse (RN) to serve as a Hospital Liaison. In this role, you will provide both clinical and administrative oversight at your assigned hospital. You will develop a hospice presence within the hospital, attend and facilitate care conferences, and work closely with healthcare providers to increase hospice admissions. You will also perform hospice admissions as needed.
This position requires a commitment to the hospice philosophy of care and the ability to collaborate with a diverse patient population. The ideal candidate will be a strong communicator who can maintain professionalism while showing care and compassion.
Identify hospitalized nursing home residents who meet hospice eligibility and arrange or conduct admissions before discharge.
Develop and maintain positive relationships with referral sources, promoting exemplary customer service to physicians, facility staff, patients, and families.
Collaborate with hospital case managers to review the hospital’s daily census and identify appropriate patients for hospice admissions.
Perform hospice admissions as needed and serve as a covering visit nurse for hospitalized patients in the absence of regular staff.
Provide consultations and follow-ups as requested by hospital case managers.
Identify and develop new referral opportunities for hospice admissions.
Participate in the on-call rotation to handle new referrals when the office is closed.
Current and valid New York State Registered Nurse (RN) license; BSN preferred.
Minimum of two years of recent clinical nursing experience, with strong clinical skills, including one year of recent Med-Surg experience.
Ability to assess the needs of terminally ill patients and their families, and to plan care to meet these needs.
Commitment to take and pass the hospice and palliative care nursing certification exam (CHPN) within three years of hire if not currently certified.
Strong commitment to the hospice philosophy of care.
Ability to relate to a diverse population and workforce in an effective, personable manner.
Excellent written and verbal communication skills.
Proficiency in Microsoft Office Suite, EMRs, and the ability to learn new software systems.
Hospice admission and end-of-life care experience is preferred.
Daily access to a vehicle with a valid driver’s license, acceptable driving record, and insurance for travel.
Bilingual (Spanish-English) candidates are a plus!
