Responsibilities:

·         Perform comprehensive nursing assessments addressing physical, emotional, psychosocial, and spiritual needs.

·         Collaborate with the interdisciplinary team to develop, implement, and evaluate personalized care plans.

·         Provide skilled nursing interventions including wound care, medication management, IV therapy, symptom/pain control, and patient/family education.

·         Deliver emotional support and education to patients and caregivers based on individual needs.

·         Supervise LPNs/LVNs and home health aides while coordinating care activities and documentation.

·         Respond to urgent needs, participate in admissions/on-call, and adjust plans based on ongoing monitoring.

·         Ensure accurate, compliant clinical records and high-quality, compassionate care delivery.

Qualifications:

  • Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
  • One (1) year experience as a RN.

Required Certifications and Licensures:

  • Licensed to practice as a registered nurse in the state of agency operation.
  • Must possess and maintain valid CPR certification while employed in a clinical role.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Ability to travel to all business locations.
