Primary Care Physician – Full-Time

Overview:
We are seeking a Primary Care Physician to provide comprehensive, patient-centered care within a collaborative interdisciplinary team. This role emphasizes building long-term relationships with a manageable patient panel, allowing providers to deliver meaningful, holistic care and directly observe the impact of their work.

Key Responsibilities:

• Conduct thorough in-person assessments and partner with the healthcare team to develop individualized care plans.

• Coordinate patient discharges for acute, short-term, and long-term care needs.

• Focus on delivering comprehensive, high-quality care to a smaller number of patients per day.

• Collaborate closely with other healthcare professionals to ensure continuity of care and optimal patient outcomes.

Candidate Requirements:

• 1–2 years of relevant clinical experience.

• Board certification preferred in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics.

• Eligibility to provide services under applicable healthcare programs.

• Strong communication, clinical judgment, and organizational skills.

• Commitment to patient-centered care and interdisciplinary teamwork.
