Physical Therapist PT – Home Health, Full-time

Join our dynamic Home Health team as a dedicated Physical Therapist, where your expertise will be pivotal in transforming the lives of patients recovering from illness, injury, or surgery. This full-time role focuses on delivering exceptional, patient-centered care directly in the comfort of their homes.

Key Responsibilities:

• Conduct comprehensive physical assessments and review medical histories to evaluate patients’ conditions.
• Create and implement personalized treatment plans tailored to each patient’s unique needs and objectives.
• Administer therapeutic exercises and training programs to enhance patient recovery.
• Provide education to patients and their families on injury prevention, wellness, and the recovery process.
• Monitor, document, and report on patients’ progress towards their rehabilitation goals.
• Collaborate with multidisciplinary healthcare teams to ensure comprehensive patient care.
• Maintain accurate and compliant patient records and documentation.

Qualifications:

• Master’s degree in Physical Therapy.
• Minimum of 1 year of experience in home health as a physical therapist.
• Current physical therapy license within the state of practice, adhering to professional standards and practices.
• Valid CPR certification, maintained throughout the clinical tenure.
• Licensed driver with the ability to travel to various business locations.
