Physical Therapist, PT Home Health – Full-time

We are seeking a dedicated and experienced Physical Therapist to join our team providing home health services in Lebanon. This full-time position offers the opportunity to make a significant impact on patients’ lives by delivering personalized therapy in the comfort of their own homes.

Key Responsibilities:

• Conduct initial and ongoing assessments of patients’ physical conditions.
• Develop personalized treatment plans to improve mobility, function, and overall well-being.
• Implement therapy sessions using evidence-based techniques and modalities.
• Educate patients and their families on treatment plans and exercises to be performed at home.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Maintain accurate and timely documentation of patient progress and treatment outcomes.

Qualifications:

• Master’s degree in Physical Therapy.
• At least 1 year of experience as a physical therapist in a home health setting.
• Valid state licensure to practice as a physical therapist, meeting professional standards.
• Current CPR certification required during clinical employment.
• Valid driver’s license with the ability to travel to different client locations.
