Key Responsibilities:

  • Perform comprehensive physical assessments and analyze medical histories to evaluate patients’ conditions accurately.
  • Create and implement tailored treatment plans that address individual rehabilitation goals and patient needs.
  • Conduct therapeutic exercises and training programs to improve mobility and enhance overall functionality.
  • Educate patients and their families about injury prevention, wellness strategies, and the recovery journey.
  • Monitor and document patients’ progress, adjusting treatment plans as necessary to achieve rehabilitation goals.
  • Collaborate with a multidisciplinary healthcare team to ensure holistic patient care.
  • Maintain meticulous patient records and documentation, complying with regulatory standards.

If you bring the following, we’d love to connect:

  • Skilled in implementing individualized care plans and communicating patient findings, progress, and changes effectively to the Physical Therapist and treating physician
  • Experienced in educating patients and families on home safety protocols and proper medical equipment use

Required Certifications & Licensures

  • Associate’s degree from an accredited Physical Therapy program
  • Active and valid license, registration, or permit to practice in the state of agency operation
  • Current CPR certification, maintained continuously throughout employment in a clinical role
  • Valid driver’s license with the ability to travel across all business locations as needed
