The Patient Care Manager (PCM) is responsible for the episode of care for the assigned team patient census with a
focus on direct management and leadership of associated team (Clinical Support Specialist & Field Clinicians).
Job Description:
As a Patient Care Manager, you will:
- Responsible for communication & coordination of care needs reported by case managers (driveway calls).
- Ensure schedules are updated, continuity of care for patient episode is present, and that quality care is delivered
by the clinical care team.
- Facilitate Case Conference and be responsible for follow-up action related to case conference, stand up, and
stand down meetings.
- Coach and mentorship of clinicians and CSS role – including monthly 1:1 session to address development needs
and hold accountable to role specific Key Performance Indicators (KPIs). Support development of new hires and
tenured employees.
- Responsible for voluntary attrition of assigned team, acting as the main point of contact for support and working
develop solutions to potential personnel concerns.
- Supervising and directing the work of all team members, assigning of admit and evaluation visits, scheduling visits and ensuring staff are completing visits in a timely manner
- Manage the site in the absence of the Clinical Manager or Executive Director or Administrator
- Managing and overseeing clinical and administrative and recordkeeping functions
- Responding to phone referral/inquiries in a timely, professional, and compassionate manner
- Participate in on-call nursing schedule and providing direct skilled bedside nursing care in patients homes
Requirements of Qualified Candidates:
- Registered nurse with current licensure to practice nursing in the practicing state
- Possess and maintain CPR certification
- Previous experience in home care setting with two years management or supervisory experience, preferred
- Proven ability to work within an interdisciplinary setting
- Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization
requirements and is in good working order
- Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal,
state, and local regulatory requirements
- Demonstrates excellent observation, nursing judgment, and communication skills
