Patient Care Manager / Registered Nurse, Home Health

We are seeking a dedicated and experienced Patient Care Manager / Registered Nurse to join our home health team in Desoto. As a key member of our health care team, you will lead and manage patient care operations, ensuring the highest standards of care for our patients in the comfort of their homes.

Responsibilities:

• Oversee the delivery of patient care services, ensuring compliance with all regulatory requirements and standards.
• Supervise, mentor, and support nursing staff to ensure optimal patient outcomes and staff development.
• Coordinate with interdisciplinary teams to develop and implement individualized patient care plans.
• Maintain accurate and timely documentation of patient care and progress.
• Conduct regular reviews and audits of patient records and care plans to ensure quality and compliance.
• Act as a liaison between patients, families, and healthcare providers to facilitate communication and care coordination.
• Participate in continuous quality improvement initiatives and professional development activities.

Qualifications:

• Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
• One (1) year experience as a RN.
• Experience in home health care or a related field.
• Proven leadership and management skills with a focus on patient-centered care.
• Strong organizational and communication skills.
• Ability to work independently and collaboratively in a fast-paced environment.
• Proficient in electronic medical records and healthcare management software.
