Patient Care Manager / Registered Nurse, Home Health

We are seeking a dedicated and experienced Patient Care Manager / Registered Nurse to join our Home Health team in College Station. This role is pivotal in ensuring the highest quality of patient care and requires a leader who is committed to excellence.

Key Responsibilities:

• Oversee and manage patient care services, ensuring compliance with all applicable regulations and standards.
• Coordinate and supervise the delivery of high-quality home health services, working closely with a team of healthcare professionals.
• Develop individualized patient care plans and continuously monitor their effectiveness.
• Provide leadership and guidance to nursing staff, fostering an environment of continuous improvement and professional development.
• Ensure accurate and timely documentation of patient care activities.
• Act as a liaison between patients, families, and healthcare providers to facilitate communication and address any concerns.

Qualifications:

• Graduated from an approved school of professional nursing.
• One year experience as a registered nurse, preferred.
• Home Health leadership experience required.
• Proven experience in a home health setting, with a strong understanding of regulatory and compliance requirements.
• Excellent leadership and communication skills.
• Strong organizational skills and the ability to manage multiple priorities effectively.
