Responsibilities:

·         Manage daily Key Performance Indicators (KPIs), including monitoring productivity, visit utilization, and quality metrics.

·         Provide clinical and operational support to field staff (RNs, therapists, LVNs, and aides), including troubleshooting, guidance, and escalation of issues.

·         Process verbal orders, coordinate workflow, and handle escalation or complaint calls efficiently.

·         Support case management by reviewing and managing patient episodes, with focus on preventing Low Utilization Payment Adjustment (LUPA) risks, appropriate paraprofessional (HHA) utilization, correct visit types, and overall episode management.

·         Report on case management activities to ensure quality outcomes and regulatory compliance.

·         Update clinical records, assist with documentation, and maintain accurate patient information in the Electronic Medical Record (EMR) system.

·         Coordinate with clinicians, interdisciplinary team members, and other departments to promote continuity of care and efficient operations.

·         Participate in quality improvement initiatives, case conferences, and performance reviews as needed.

·         Assist with care plan reviews, missed visit management, and other administrative/clinical support tasks to optimize patient outcomes and agency performance.

Qualifications:

  • LVN/LPN with current licensure to practice nursing in the practicing state.
  • Possess and maintain CPR certification.
  • Previous experience in home care setting preferred.
  • Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and is in good working order.
  • Prior use of Electronic Medical Records (EMR) preferred.
