Hospital RN Care Manager  

 Responsibilities

  •          Performs the initial comprehensive assessment on admission
  •         Provides ongoing assessment of needs
  •          Monitor effectiveness of services
  •          Track progress toward individual goals
  •          overseeing transitions for patients and assess all discharge planning needs

Qualifications

  •          BSN required
  •         New York RN license/registration.
  •          One year of Hospital Care Management experience
  •          Care Management Certification
  •          PRI Certification
