Hospice Transition Care Coordinator

We are seeking a Hospice Transition Care Coordinator to join our team!

Main Duties of position:

·         Clinical: perform needs assessments, admissions, maintain an open line of communication (customer connections/clinical team/sales teams/referral source), documentation due within 24hrs

·         Sales duties: maintain and build relationships with hospital and all personnel, providing education to all entities within the hospital and patient families, responsible for strategic planning for the account

Work Schedule: Monday-Friday 8am-4:30pm-one weekend a month of sales on call (making 3 sales call per weekend)

Qualifications:

v  Problem solving skills, flexibility, coachability, communication skills, professionalism

v  2 years of RN experience required.

v  Prior hospice experience and/or sales healthcare sales experience.
