Hospice RN

Hospice Registered Nurse (RN)

Location: Richmond VA

Responsibilities:

• Provide skilled nursing care and administer medications in patients’ homes
• Complete assessments and develop care plans with the hospice team
• Monitor and report changes in patient condition
• Participate in interdisciplinary team (IDT) meetings
• Document all care using an electronic charting system
• Educate and support family members and caregivers
• Participate in on-call and weekend rotation as needed

Qualifications:

• RN license in Virginia
• 1–2 years of RN experience required (hospice or home care preferred)
• Basic computer/iOS skills
• Strong communication, compassion, and teamwork skills
