Home Health RN, Registered Nurse – Per-Diem

We are seeking a compassionate and skilled Home Health RN, Registered Nurse to join our team on a Per-Diem basis in San Gabriel. The ideal candidate will provide high-quality care to patients in their homes, ensuring comfort and promoting their well-being.

Key Responsibilities:

• Conduct comprehensive assessments of patients’ health status, including physical, social, and psychological aspects.
• Develop and implement personalized care plans in collaboration with patients and their families.
• Administer medications and treatments as prescribed, maintaining accurate and detailed records.
• Monitor patient progress and adjust care plans as necessary to achieve optimal outcomes.
• Educate patients and their families on health management, disease prevention, and safety protocols.
• Coordinate with interdisciplinary teams to ensure cohesive and effective patient care.
• Maintain compliance with all health regulations and standards of practice.

Qualifications:

• At least one (1) year of experience as an RN
• Home Health experience as a Registered Nurse required.
• Valid California RN license required
• Current CPR certification required 
• Current California driver’s license, good driving record, and reliable transportation
• Located in Southern California
