In this role, you will work closely with patients and their families to assess health conditions, develop care plans, administer treatments, and ensure the best possible outcomes for individuals who may have chronic illnesses or are recovering from surgeries. As a crucial link between patients and the healthcare system, you will utilize your nursing skills and clinical knowledge to educate patients on managing their conditions and maintaining their overall health. You will be responsible for monitoring patient progress, coordinating with other healthcare professionals, and adapting care plans as necessary. Responsibilities

  • Conduct comprehensive patient assessments to determine healthcare needs.
  • Develop individualized care plans in collaboration with patients and families.
  • Administer medications and treatment as prescribed by physicians.
  • Monitor and document patient progress and response to treatment.
  • Educate patients and caregivers on disease management and health maintenance.
  • Coordinate with interdisciplinary teams to ensure comprehensive patient care.
  • Respond to patient inquiries and provide support in a timely manner.

Qualifications

  • Active (unrestricted) nursing license in the State of California
  • 1 year (professional) nursing experience in Home Health or Acute Care
  • Valid Driver’s License w/ Clean DMV
