Home Health – RN, Registered Nurse

We are seeking a dedicated and compassionate Registered Nurse (RN) to join our Home Health team in Medford. This role presents an opportunity to make a meaningful impact by providing high-quality care to patients in the comfort of their homes.

Key Responsibilities:

• Conduct comprehensive assessments of patients’ health status and develop individualized care plans.
• Provide nursing care and services to patients according to the plan of care and physician’s orders.
• Educate patients and their families on health maintenance and disease prevention.
• Collaborate with other healthcare professionals to ensure coordinated and effective patient care.
• Monitor and document patient progress and report changes to the healthcare team.

Qualifications:

• Current and valid Registered Nurse (RN) license in the state of Massachusetts.
• Bachelor’s degree in Nursing or related field preferred.
• Minimum of 1 year of experience in home health or related healthcare setting.
• Strong clinical assessment and communication skills.
• Compassionate, patient-focused, and able to work independently.
• Valid driver’s license and reliable transportation.
