We are seeking a dedicated and compassionate Home Health Registered Nurse (RN) to join our team in Burnsville. As a Home Health RN, you will play a critical role in providing high-quality, patient-centered care to individuals in their homes, promoting their independence and well-being.
Key Responsibilities:
- Conduct comprehensive assessments of patients’ health status and develop individualized care plans.
- Administer medications, treatments, and procedures as prescribed by the physician.
- Monitor patients’ progress and adjust care plans as necessary to ensure optimal health outcomes.
- Educate patients and their families on managing illnesses or injuries and promote healthy lifestyle choices.
- Collaborate with other healthcare professionals to coordinate patient care and ensure continuity of services.
- Maintain accurate and timely documentation of patient care activities and progress notes.
- Ensure compliance with all state and federal regulations and standards of care.
Qualifications:
- Current and unrestricted RN license in the state of Minnesota.
- Bachelor’s degree in Nursing (BSN) preferred.
- Minimum of one year of nursing experience.
- Require Home Health Experience.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Reliable transportation and valid driver’s license.
