Home Health Occupational Therapist
· Requirements:
o Must have Ohio Occupational Therapist License
o Must meet productivity expectation of 15 points per week
o Home health experience required
· Job Description:
o Patient Assessment: Evaluating a patient’s functional abilities, limitations, and needs in their home environment.
o Treatment Planning: Developing individualized treatment plans with specific goals and objectives to improve daily living skills and independence.
o Intervention: Providing interventions to help patients regain function and improve their ability to perform daily activities.
o Education: Educating patients and caregivers on techniques for improving daily living, safe home modifications, and using assistive devices.
o Documentation: Maintaining accurate and timely documentation of patient assessments, treatment plans, progress notes, and discharge plans.
o Communication: Collaborating with other healthcare professionals, such as physicians, physical therapists, and nurses, to ensure coordinated care.
o Discharge Planning: Developing discharge plans to ensure a smooth transition back home or to other care settings.
o Supervision: Supervising therapy assistants and aides when appropriate
· Additional information:
o 13 week contract with possible extensions (20 hours per week)
