Director of Case Management

Role Responsibilities:

• Oversee Case Management operations, including discharge planning, patient advocacy, and resource management.
• Collaborate with healthcare providers to optimize care transitions and performance improvement initiatives.
• Educate and mentor staff on Case Management standards and regulatory compliance.
• Lead efforts to improve care coordination across the healthcare continuum.

Qualifications:

Must-Haves:

• Associate’s Degree in Nursing
• Current RN license in NC or multi-state (compact) license
• BLS Certification
• CCM Certification (within first year)
• Minimum 3 years RN or Care Coordination experience (acute care)

Preferred:

• Bachelor’s or Master’s Degree in a related field
• ACMA Certification
• Minimum 3 years management experience
