Care Management Services Director

About the Role:
This leadership role oversees care management services and drives patient care facilitation across multiple hospital departments.

Roles and Responsibilities:

• Develop, organize, and manage care facilitation services (e.g., Case Management, Social Work, Utilization Management, post-acute care coordination)

• Lead Hospital Case Management programs to improve patient progression and physician collaboration

• Shape patient care experiences through effective care-plan interventions

Job Requirements:

• Current state RN License

• 5 years of case management experience in a healthcare setting

• Strong communication and customer relations skills

Preferred Qualifications:

• Master’s degree

• 3-5 years of supervisory experience

• Formal case management training
