TITLE: Antimicrobial Stewardship Pharmacist

What they’re looking for:
•Pharmacy doctorate (PharmD) or equivalent. Residency preferred.
•Licensed Pharmacist in the State of Ohio.
•At least two years experience in hospital or similar health system.
Hours and compensation potential:
• The position is full-time day shift.
• The range is between for $108K-149K depending on experience.
• Full benefits package offered as well.
What you’ll be doing:
•Provides general clinical pharmacy services, which include M.U.E. development and reporting, some teaching opportunities, as well as clinical staffing.  
•Leads antimicrobial stewardship strategy for accountable site(s) in collaboration with system antimicrobial stewardship team. 
•Develops, enforces and maintains policies and procedures that promote and enhance patient outcomes through safe, effective, cost-efficient and appropriate patient medication therapy use for patients receiving antimicrobials. 
•Coordinates and maintains the antimicrobial stewardship program including:
    a. Overseeing utilization audits of antimicrobial therapy.
    b. Assisting in development and maintenance of an antimicrobial formulary approved by the P&T committee.
    c. Work with the infectious disease service and hospital medical staff to evaluate appropriate use of restricted antimicrobials.
    d. Develop guidelines, policies, electronic health management solutions and other monitoring and intervention strategies for antimicrobial use and streamlining prophylactic, empiric and definitive antimicrobial therapy.
    e. Review data and metrics on antimicrobial use and report to the infectious disease committee and P&T committee.
    f. Coordinating, facilitating and promoting education, training and competency assessment and performance improvements of pharmacists in the appropriate use of antimicrobials.
    g. Serves as pharmacy consultant to infectious disease committee and P&T committee.
•Responsible for ensuring compliance with regulatory standards related to safe and appropriate antimicrobial use. 
•Contacts prescribers to clarify medication orders or notify them of medications that are unobtainable to reduce the possibility of medication errors or delay in therapy.
•Assists in the inventory and ordering of pharmaceuticals so that stock outages are minimized, and appropriate inventory levels are maintained. 
•Helps evaluate processes that utilize material resources in order to provide cost effective care within budget. 
• Assists the Purchasing Associate in procuring quality generic products as well as supports the formulary practices of the department to provide optimal care at the most affordable price. 
•Maintains medication on assigned patient care areas by initialing inspection forms and submitting reports to the manager.
•Promptly prepares emergency carts and boxes to assure adequate quantities of in-date medication are available for emergency situations. 
•Calculates caloric and/or electrolyte ingredients for TPN resulting in correct administration of patients’ nutritional therapy. 
•Monitors drug therapy of patients by screening for potential medication problems so that issues involving drug interactions, incompatibilities, and potential allergies are resolved without patient harm. 
•Applies pharmacokinetic/pharmacodynamic principles to individualize drug dosing for patients.
•Responds to and supports Code Blue and Code Trauma situations to maximize opportunity for patient survival and stabilization.
