After Hours – Registered Nurse, Hospice: On-call, 7 on 7 off

Ideally, someone in the following areas or able to travel to: Central Marion County, North or Northeast

JOB SUMMARY:

The after-hours registered nurse supports the patient needs of the agency in the evening during the weekdays and over the weekend.  The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities after normal business hours. 

 

You begin each evening at 4:30pm, but then cover Friday 4:30pm to Monday at 8:00am.  

 
You will have a backup RNCM, Supportive care team member and administrator on call with you each day.  You will be one of two After Hours, RN’s that alternate weeks.  We also have a weekend LPN that support you over the weekend.
 
We are looking for a compassionate, talented and experienced Hospice On-Call Nurse (RN) that is eager to join an exciting organization and become part of our Hospice On-Call RN team.  
 
The RN, On-Call position provides service for the personal, physical, emotional and social needs of the patient-family unit. RN will be responsible for evaluating and responding to the patient’s/caregiver’s needs via telephone contact, home visits, and/or facility visits for assessment, symptom management, treatment, teaching and coordination of care.

JOB REQUIREMENTS & QUALIFICATIONS:

Bachelor’s or Associate’s degree in nursing

Maintains an active CPR certification.

At least 2-5 years of experience in the healthcare management, hospice, or palliative care and/or training in end-of-life care, preferred.

At least 1 year of experience with Medicare Hospice Certification as well as CHAP guidelines, preferred.

Knowledge of hospice care and the services provided to patient and family/caregiver through an interdisciplinary group.

Proficient in MS Office applications and ability to learn department and job-specific software systems.

Demonstrate organizational skills.

Demonstrate effective verbal and written communication skills.

Demonstrate analytical skills when problem-solving.

Demonstrate high attention to detail and a high degree of accuracy.
