As a Psychiatric Nurse Practitioner, you will evaluate, diagnose, and treat patients with mental, emotional, and behavioral disorders. They conduct thorough psychiatric evaluations, develop treatment plans, prescribe medication, and evaluate treatment results. Expected to provide excellent patient care and compassion in every encounter and across programs, aligned with the organizations core values.

What your day might look like:

  • Prescribes, directs, or administers psychotherapeutic treatments or medications to treat mental, emotional, or behavioral disorder
  • Orders, analyzes and evaluates patient data or test findings to diagnose nature or extent of mental disorder
  • Promotes positive treatment outcomes and client safety by providing tele-health and/or in- person services as directed by clinic leadership and/or emergency response plan
  • Provides tele-health or in-person services across program locations, including both the Glens Falls and Plattsburgh regions;]
  • Completes training and provides treatment using Medication Assisted Therapies (MAT) as part of an evidence-based integrated approach to delivering behavioral healthcare services
  • Collaborates with an integrated team to discuss treatment plans and progress
  • Performs psychiatric assessments and evaluations
  • Designs, executes or approves individualized care plans, using a variety of treatments
  • Gathers and maintain patient information and records
  • Documents all services within twenty-four hours of the date of a client’s visit and in accordance with state/federal guidelines and company policies and procedure
  • Participates in care planning and coordination meetings to facilitate care process as requested. As assigned, sees individuals who are an immediate crisis to curtail emergency visits and/or inpatient admissions
  • Participates in quality assurance and quality improvement activities
  • Maintains client confidentiality and confidential health information
  • Participates in meetings as scheduled
  • Meets or exceeds the then current individual billable units of service requirement
  • Additional duties as assigned.

Your skills and qualifications:

  • Current and valid licensed Psychiatric Mental Health Nurse Practitioner / PMHNP or transferable license to NYS
  • Previous experience providing psychotherapy to adults and/or children preferred
  • Previous training in and experience with providing Medication Assisted Therapies (MAT) preferred
  • Licensed to practice in state of practice
  • Current DEA registration
  • Valid NYS driver’s license with driving record acceptable to insurance carrier and reliable transportation required for required travel to other agency locations in Upstate NY region
  • Extensive knowledge with PC applications and electronic medical records strongly preferred.
