Occupational Therapist Home Health

Responsibilities:

• Initial Evaluation pf patient’s functional status in a home setting
• Development and implementation of a plan of care
• Ongoing one-on-one treatment in the home
• Educate patients and caregivers on the plan of care and individualized treatment plan
• Maintains and submits all clinical documentation in compliance with Agency standards

Requirements:

• Occupational Therapy License New York State
• Minimum one year clinical practice. Home Care preferred
• Valid NYS Drivers License and reliable transportation
• Travel throughout the Borough of Queens
