Hospice Social Worker

Hospice Social Worker part-time or full-time

Responsibilities:

• Provide Clinical Social Work to Hospice population.
• Complete initial and ongoing social evaluation of patient and family dynamics
• Participates in the coordination of services with other hospice team members and community resources as needed.
• Maintains and processes all documentation in compliance with agency standards.

Requirements:

• MSW degree in Social Work
• Licensed Social Worker in the state of Massachusetts
• Minimum 1 year of clinical experience; Home Care or Hospice highly preferred
• Valid Driver’s License, auto insurance and reliable transportation
• Compassion and integrity
