Hospice Licensed Independent Clinical Social Worker part-time
Responsibilities:
- Provide Clinical Social Work to Hospice population.
- Complete initial and ongoing social evaluation of patient and family dynamics
- Participates in the coordination of services with other hospice team members and community resources as needed.
- Maintains and processes all documentation in compliance with agency standards.
Requirements:
- LICSW Licensed Independent Clinical Social Worker (LICSW) in New Jersey
- Home Care, Hospice or Bereavement counseling experience
- Valid Driver’s License, auto insurance and reliable transportation
- Compassion and integrity
